bullet ADULT DAY PROGRAMS & SERVICES

Community Habilitation Asst. Mgrs. – P/T & F/T (Hauppauge)
Responsible for the implementation of individualized services in all Community Habilitation areas emphasizing community integration and in-home activities. Provide individuals served with activities that support their chosen outcomes to live, work, volunteer, socialize and participate within their community with maximum independence; Complete daily and monthly data/reports and assist in the development of individuals service plans. Associates Degree or equivalent and one year experience with working with individuals with developmental disabilities in the capacity required by essential job duties; Or three years experience working directly with individuals with developmental disabilities in the capacity required by essential job duties. Current valid NYS Drivers license; Obtain and maintain First Aide/CPR Certification (within probationary period). Others as required by regulation changes.

Program Aides – F/T (Hauppauge)
Assists in implementation and recording of program services for individuals as outlined in each individual’s plan of care. Responsible for the protective oversight and safeguards of the individuals assigned throughout the program day. High school diploma or general education degree (GED) required. SCIP-R, AMAP and CPR certification preferred. Clean and valid NYS Drivers License required.

Day Habilitation Assts. – F/T (Hauppauge & Bohemia)
Responsible for the implementation of individualized services in all Day Habilitation areas emphasizing community integration and in-house activities to enhance the People Served in their chosen outcomes affecting his/her capacity to live, work, socialize, and participate within the community. High school diploma or general education degree (GED); or one to three months related experience and/or training. Valid New York State Driver's License required and Valid First Aid Certification necessary.

bullet EDUCATION

Speech Language Pathologist - F/T 12 month (Commack)
The Speech-Language Pathologist is responsible for the diagnosis and treatment of speech-language, pre-speech and feeding disorders for communicatively impaired individuals as assigned by the Clinical Supervisor. Master's degree (M. A.) or equivalent and three to five years of related experience and/or training. Certification by the American Speech/Language Hearing Association. New York State Licensure.New York State Department of Education Certificate as Teacher of the Speech and Language Handicapped.

Teacher of the Visually Impaired - P/T or F/T (Commack)
Provides appropriate individualized instruction to meet the needs of students according to their Individualized Family Service Plans and Individualized Education Plan. Evaluates and assesses students with visual impairments and maintain appropriate record s. Minimum, Bachelor's degree (B. A.), preferably a Master of Science degree from a four-year college or university; some experience preferable. NYS Certification as a Teacher of the Visually Impaired required.

Teacher Subs (Commack)
Responsible for the education of each student on a day-to-day basis. Minimum, Bachelor's degree (B. A.), and a Master of Science degree, from four-year college or university. Previous teaching experience is preferred. NYS teaching certification in Special Education or NYS Teaching Certification in another area (i.e., Elementary Education, Social Studies, etc.) or Bachelors degree in the subject matter and working towards a NYS teaching certification.

Teacher Aides Subs (Commack)
Assists classroom teacher in curriculum implementation by working with individuals or groups of children as assigned by classroom teacher. Assists in implementing strategies suggested by clinicians to the teacher. Performs frequent daily lifting of individuals with disabilities (ages 2-21), for their activities of daily living needs. High School Diploma or general education degree (GED) required.

Per Diem RNs (Commack)
Provide first aid and emergency treatment for the students. Administer medications and treatments as prescribed. Provide tuberculi testing. Monitor/liaison between private duty nurse and program. Perform all other duties as may be assigned by the Principal/designee. NYS license to practice as a registered nurse. Nurse required.

bullet ADMINISTRATION

Special Events Coordinator F/T (Hauppauge)
Plans and implements all aspects of small fund raising events. Works closely with committee members as well as recruits members and volunteers and secures sponsorships. Prepares reports, mailings and all necessary printed material for committee meetings and events. Represents UCP Long Island at events and community activities. Responsible for reconciliation of event revenue and works in accordance with budgets. Bachelor’s degree (B.A) required and 1-3 years related experience. Microsoft and Razor’s Edge software experience necessary.

Residence Managers (Various locations)
Manages one subordinate supervisor (Assistant Residence Manager) who supervises a total of up to 13 employees in the Residence. Responsible for the overall direction, coordination, and evaluation of these units. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to three years related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Assistant Residence Managers (Various locations)
Assist Residence Manager to perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities. Function as Manager in Manager's absence. Supervises a total of up to 13 employees in the Residence. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to six months related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Registered Nurses - F/T, P/T & Per Diem (Various locations)
Responsible for monitoring and assisting in the overall health and safety of the individuals served. Insure that all indicated medical appointments are scheduled and followed up appropriately. Insure maintenance of all medical records per state and federal guidelines. Attend annual/interim meetings, weekly IDT meetings, and staff meetings. Participate in supervision and in-service training of direct care staff. Meet regularly with residence managers to discuss current health care concerns (minimum once weekly). Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience. New York State - Licensed Registered Nurse required.

Social Worker – F/T (Various locations)
Advocate for all participants of ICF programs and act as a liaison with state and federal agencies, parents, guardians, day treatment and habilitation programs. Will be part of an interdisciplinary team providing a spectrum of services which will be provided to the residents through the use of social work methods directed toward maximizing the social functioning of each resident. Coordinate and insure communication with OPWDD and the Community Advisory Board. Master's degree (M. A.) in Social Work and one to three years related experience and/or training. Valid and current Licensed Master Social Worker (LMSW) license.

Recreation Therapists F/T, P/T (Contractor-Various locations)
Plan a structured recreation program which will be incorporated into each individuals daily activity schedule including treatment goals and objectives that will be developed, monitored and updated as needed; Create a monthly recreation calendar of activities for use by the residence to provide information and outings for program participants and staff. Responsible for annual evaluation of individual’s strengths and needs; annual evaluation of the individual’s level of functioning in reference to recreational skills; annual assessments; interim reviews; monthly summaries; monitoring data sheets in reference to recreation goals and objectives. Supervise, instruct, and demonstrate programs and activities to the staff that will implement the program, and provide follow up to monitor staff and programs. Bachelor's degree (B. A.) from four-year College or university and one to six months related experience.

Provide nutritious meals, box lunches and snacks in regulation with the special dietary needs of the participants. Insure cleanliness and proper sanitation of ICF. This includes resident’s bedrooms, dining room, nursing station, corridors, lobby, lounge, recreation room and laundry room. High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

Cook (P/T) (Commack)
Provide nutritious meals, box lunches and snacks in regulation with the special dietary needs of the participants.. High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

Direct Support Professionals - F/T, P/T & Per Diem (Various Locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid NYS Driver’s License.

FACILITIES

Maintenance Mechanic F/T (Various locations)
Maintain all residential facilities. Maintain each facility in a clean, comfortable and safe environment for participants and staff. Perform manual work in building repair and grounds care. Assist in the control of snow and ice removal. High school diploma or general education degree (GED) and one to three months related experience and/or training. Must have a minimum one-year experience in housekeeping or similar field. Valid current New York State Driver License required.
